• Description: Provides clients with access to basic needs supplies such as water, food, ice, clothing, chargers, survival gear, and hygiene supplies as needed. The Street Outreach Team visits local encampments and known areas of residence for current and potential clients. The team also assists with resource navigation including transportation to and advocacy for medical appointments, Department of Employment and Social Services, DMV, Social Security, court dates, free phones, and housing and employment searches. The Street Outreach Team meets clients where they are to help eliminate barriers to reaching their self-defined goals.

• Hours: The Street Outreach Team will be out in the community Monday through Friday. Case management is available on site by appointment only.
• Eligibility: Individuals experiencing homelessness or anyone experiencing housing or economic insecurity.
